Clinical Care Coordinator (LVN)

Renew Home Health is looking for an organized, compassionate Clinical Care Coordinator (LVN) to help keep patient care running smoothly across our Cleburne office. In this full-time role, you'll be the link between patients, families, clinicians, and physicians — coordinating schedules, orders, and communication so every patient gets the right care at the right time. What you'll do as a Clinical Care Coordinator (LVN):
- Coordinate patient schedules, visits, and clinician assignments
- Communicate with patients, families, physicians, and referral sources to keep care on track
- Process and follow up on physician orders and documentation
- Support the clinical team with timely, accurate charting and records
- Help ensure care is delivered in line with the plan of care and compliance standards
- Keep the office running smoothly and jump in wherever the team needs you
What we're looking for
- Current Texas LVN license in good standing
- Home health experience a plus (coordination or scheduling experience especially welcome)
- Strong organizational and communication skills
- Comfortable with technology and electronic documentation
- A team player who stays calm and organized under pressure
