What you’ll do as a Home Health Admit RN: As a PRN admit nurse, your primary focus is new admissions and readmissions across the Fort Worth area, with occasional routine visits when the team needs extra coverage. Typically, your work includes: - Completing thorough start-of-care and resumption-of-care assessments with OASIS documentation - Establishing the initial plan of care with the physician - Educating patients and families at the start of their care - Coordinating with the clinical team for a smooth hand-off to ongoing visits - Stepping in on routine visits when needed - Documenting each visit accurately and on time
What we're looking for
What we are looking for: - First, you need an active Texas RN license. In addition, we look for the following: - Home health experience with admissions and OASIS / start-of-care (strongly preferred) - A valid Texas driver’s license and reliable transportation - Strong assessment, communication, and time-management skills - Availability for PRN / as-needed visits - A caring, patient-first attitude
