- Assess each patient’s psychosocial needs and the social factors affecting their health and recovery
- Connect patients and families with community resources — financial assistance, transportation, meals, caregiver support, and long-term care options
- Provide short-term counseling and emotional support
- Help with advance directives, care planning, and safe transitions of care
- Coordinate with the nursing and therapy team on each patient’s plan of care
- Document each visit accurately using our AI-assisted charting tools
What we're looking for
- Master’s degree in Social Work (MSW) from a CSWE-accredited program
- Current Texas social work license (LMSW or LCSW)
- At least one year of social work experience in a healthcare setting (home health experience a plus)
- A valid Texas driver’s license, reliable transportation, and current auto insurance
- Strong communication skills and a caring, patient-first attitude
