Hospice Clinical Liaison (RN)
About the role
A well-established hospice provider serving The Woodlands and the North Houston corridor is hiring a Clinical Liaison. This is a field role for an RN who would rather be in front of referral partners and families than charting at a desk. You carry the clinical credibility that closes referrals, and you own the territory that generates them.
What you'll do:
- Develop and grow referral relationships across hospitals, skilled nursing facilities, assisted living and memory care communities, physician practices, and discharge planning teams throughout The Woodlands, Conroe, Spring, and North Houston.
- Conduct bedside and in-home consultations with patients and families, explaining the hospice benefit clearly and compassionately.
- Assess referrals against Medicare hospice eligibility guidelines, review medical records, and communicate findings to the clinical team.
- Manage the admission from first call to signed consent, coordinating with intake, the admitting nurse, and the attending physician.
- Build and execute a territory plan against monthly admission and census targets.
- Serve as the clinical resource for referral partners through in-services, education, and answering the eligibility questions that stall a referral.
- Track activity in the CRM and report on referral trends, competitor movement, and account health.
- Represent the agency at community events, health fairs, and professional association meetings.
- Operate in full compliance with the Anti-Kickback Statute and all hospice marketing regulations.
What's offered:
- Competitive base plus a performance bonus tied to admissions
- Mileage reimbursement
- Medical, dental, and vision
- PTO and paid holidays
- 401(k)
- A territory with room to grow and a clinical team that answers the phone
What we're looking for
- Current, unrestricted Texas RN license (required)
- 2+ years of nursing experience; hospice, home health, or palliative care strongly preferred
- Working knowledge of Medicare hospice eligibility criteria and the certification process
- Prior liaison, admissions, business development, or territory sales experience preferred
- Comfort presenting to clinicians, administrators, and families alike
- Valid Texas driver's license, reliable transportation, and current auto insurance
- Self-directed and organized, able to run your own week without being managed
